Total Sports Release and Waiver


Every Total Sports Participant MUST agree to the Total Sports Complex waiver in order to participate in activities at any of our facilities or satellite locations (including but not limited to Total Sports Complex Novi, Total Roller Hockey, Total Sports Complex Exec, and Total Sports Complex Wixom). A parent/guardian signature is required for all players under the age of 18.

Parent/Guardian requirements:

In order to accept the online waiver, parents/guardians must:

1) Have a valid email address on file in our system.

2) Have a birthdate listed in our system.

3) Reflect all applicable minors under their name in our system.

ELECTRONIC WAIVER INSTRUCTIONS:

All players will be required to submit an electronic waiver, regardless if you have a paper waiver on file. Follow the instructions below to fulfill this requirement.

EXISTING PLAYERS/MEMBERS:

Customers that currently have a membership can click on the tab below to fill in your username and password. Your username will be the email address we have on file for you. If you do not have a password, or have forgotten yours, you can request a new one to be sent to you.

NEW PLAYERS/MEMBERS:

Click below to visit our NEW Customer registration page. Enter your email address and click sign up. Complete the form and submit. A confirmation email will be sent to your email box. You will need to log in to your email to activate your new account. Once activated, you can log in at www.totalsportscomplex.com to view your team information (schedule, standings, and payment history) or sign up for new activities!
